The Youth Policy Institute is currently seeking a Senior Case Manager for its GEAR UP program. GEAR UP stands for Gaining Early Awareness and Readiness for Undergraduate Programs, and is federal grant through the US Department of Education that follows students with programming from their 6th grade year to their first year in college. The group of GEAR UP schools are a combination of charter, pilot, and traditional LAUSD schools located in the Pacoima Los Angeles Promise Neighborhood, whose mission is to transform the communities of Hollywood and Pacoima with a continuum of cradle:to:college:and:career services, and will be a zone of choice for families with a wide range of high:quality school options. The centerpiece of the Los Angeles Promise Neighborhood (LAPN) is an innovative public:private partnership that blends diverse funding streams around a core set of outcomes and objectives. Working in collaboration with the Assistant Director of Academic Support, the GEAR UP Senior Case Manager will train, coach, and manage staff, while developing and implementing the case management component of the GEAR UP program.The Senior Case Manager will identify resources and services in the community to address family needs, supervise a case management team, and provide ongoing support for the GEAR UP team. The Senior Case Manager will oversee case file review and implementation of clientA?s goal plans, and will also track progress towards GEAR UP case management targets and benchmarks. Responsibilities may include but are not limited to:Meet regularly with Assistant Director of Academic Support and GEAR UP Manager to provide feedback, address any issues, and provide ideas and solutions.Supervise and oversee 2 full:time case managers and intake/registration process.Ability to implement a youth:centered, trauma informed and strength based model of practice.Oversee accuracy of client file, conduct file review on a weekly basis and provide feedback to case managers.Maintain systems for tracking client logs for client follow:up and intervention.Act as the forefront for the GEAR UP case management component.Motivate and encourage team work among case managers and staff.Provide and maintain appropriate data and files of the children and families.Participate in and lead staff meetings, monthly trainings and other department programs as requested.Participate in all appropriate sessions, trainings and meetings.Assist and prepare for site visits and program evaluation.Assist in development and coordination of special assignments and programs.Oversee assessment of needs and implementation of services for each plete case plans and other documentation using project protocol.Provide home and/or school visits monthly per family or as prescribed by case plan or as needed.Assist in program evaluation.Develop a personalized case plan by working with the youth to meet the goals of the case plan and oversee development of case plans of case managers.Maintain a professional attitude, confidentiality, and respect for cultural differences.Maintain a light load of intense case management clients.Will work with individual families to conduct in depth case management needs assessments to identify needs for each family member and to also reduce barriers for the familyA?s employment, health and overall well:being.Will support clients accessing services by developing a personalized case plan by working with the family to meet the goals of the case plan.Handle all other duties as assigned.May be required to travel between work sites to attend trainings, meetings, etc.Bachelors degree in Social work or related field required, Masters degree in Social Work preferred; plus 3:5 years experience providing case management services.2+ years in supervision/management role.4+ years of experience providing case management services.Strong public speaking skills.Excellent communication and organizational skills; strong written and interpersonal skills.Bilingual English/Spanish r
